Your use of a BLOCKBUSTER GiftCard® constitutes your acceptance of the complete terms and conditions.
GiftCards may be applied towards the cost of any eligible merchandise sold or rented at any participating BLOCKBUSTER® store
located in the United States up to the remaining value, except that GiftCards cannot be used to purchase Blockbuster GiftCards OR
any third party gift card. GiftCards with pin numbers may also be redeemed online at www.blockbuster.com. Membership
rules and certain restrictions apply for rental. Giftcards are not redeemable for cash, except where required by law, and may not be returned.
Lost, stolen or damaged BLOCKBUSTER L.L.C. GiftCards ("GiftCards") may be replaced with a new GiftCard in the amount of any remaining balance on the GiftCard
on the date of receipt of a completed Lost, Stolen or Damaged GiftCard Report Form ("Report Form") by Blockbuster L.L.C. ("Blockbuster").
GiftCard Number is required for replacement. If your GiftCard has been lost, stolen or damaged, please complete and sign this Report Form and mail
the completed Report Form along with an original BLOCKBUSTER store receipt and a copy of a valid government issued photo identification to
BLOCKBUSTER Customer Care Department at: BLOCKBUSTER L.L.C. Customer Care, 5701 S. Santa Fe Dr., Littleton, CO 80120.
Terms and Conditions
Upon receipt of the completed Report Form, Blockbuster will cancel the GiftCard identified above and issue a new GiftCard in the amount of any
remaining balance on the GiftCard on the date of receipt of the Report Form by Blockbuster. The replacement GiftCard will be mailed to the address provided above.
You must provide a valid street address. P.O. Boxes will not be accepted. Please make a copy of Report Form and submissions for your records.
Report Forms and documents submitted become the property of Blockbuster and will not be returned.
Please allow up to 8 weeks for delivery of the replacement GiftCard. Blockbuster shall not be responsible for any lost, late, illegible,
damaged, misdirected, incomplete or postage due mail. Reproduction, purchase, trade or sale of this Report Form, GiftCard numbers, and or cash
register receipts are prohibited. Fraudulent submissions may result in prosecution under applicable federal mail fraud statutes.
