An unusually heavy rainstorm brings a inordinate amount of "traffic" into the ER. Outside the doors of the hospital, Weaver (Laura Innes) and a nervous, inexperienced Gallant (Sharif Atkins) risk electrocution from downed power lines to save a pregnant woman and her unborn baby. Inside, Greene (Anthony Edwards) treats a young boy whose brother was swept into a river. Carter's (Noah Wyle) grandmother, brought into the ER after sustaining injuries in a hit-and-run, begins hallucinating. Nicole (Julie Delpy), the troubled young girl befriended by Kovac (Goran Visnjic), may be stealing personal items from the staff. And Benton (Eriq La Salle) gets some disturbing news about his son, Reese (Matthew Watkins). Originally slated to air on September 20, 2001, the eighth-season opener of ER was bumped to September 27 due to ongoing network coverage of the terrorist attacks on New York City and Washington, D.C. This inaugural episode hopscotches through time, presenting its events from several sharply differing points of view. First off, several loose ends from the end of season seven are wrapped up, notably the aftermath of Greene's (Anthony Edwards) decision to "terminate" a man who had killed several people before being shot himself, and the aftereffects of the treatment given to Cleo Finch (Michael Michele) when she cut her hand on a vial of HIV-infected blood. Kovac (Goran Visnjic) again treats the stubborn Bishop Stewart (John Cromwell). Sixteen-year-old cardiac patient Nick Stevens (Josh Peck) balks at having another heart transplant. Another patient, a gay man named Jeff (Robert Beitzel), refuses to use contraceptives, and indeed seems eager and willing to contract the AIDs virus. Greene's (Anthony Edwards) erratic behavior increases. In the first episode of a two-part story, Valentine's Day has a strange effect on Greene's (Anthony Edwards) cantankerous dad (John Cullum) and Corday's (Alex Kingston) judgmental mom (Judy Parfitt), who find themselves attracted to one another. Back at the ER, a private quarrel between Chen (Ming-Na) and Malucci (Erik Palladino) may have adverse consequences on a patient (Lenny Wolpe). A grim Kovac (Goran Visnjic) must tell two youngsters that both of their parents have been killed. Abby (Maura Tierney) treats one of Carter's patients. And Carter (Noah Wyle) brusquely brushes off Lucy (Kellie Martin) when she asks his help in dealing with a patient named Sobricki (David Krumholtz), whose "headache" may be linked to a more serious mental illness. Abby (Maura Tierney) is caught in the middle of a bitter domestic dispute that may have fatal results for a young leukemia victim in dire need of a bone marrow transplant. Kovac (Goran Visnjic) is dissatisfied when one of several people is chosen as a kidney recipient. A patient whom Carter (Noah Wyle) was forced to discharge returns in terrible shape. Returning from maternity leave, Carol (Julianna Margulies) has problems getting back with the program. Greene (Anthony Edwards) tries to nullify the abusive relationship between a gay couple. Carter (Noah Wyle) and Chen (Ming-Na) launch into another argument, this one over admitting an elderly woman (Peg Phillips) into the ER (Diane Baker guest-stars as the woman's apparently long-suffering daughter). Complications arise during a heart transplant for Lucy's (Kellie Martin) patient Valerie (Myndy Crist). Luka (Goran Visnjic) treats a group of truly colorful patients. Worn out after his first night shift since returning to the ER, Carter (Noah Wyle) is summoned back to assist in Chen's (Ming-Na) delivery. Meanwhile, the people who want to adopt Chen's baby have reason to suspect that she'll renege on the agreement. Later, Abby (Maura Tierney) and Carter share their experiences with substance abuse after he toys with the notion of using a stimulant to stay awake. And in other developments, Greene (Anthony Edwards) is in New York, hoping to find someone that can treat his brain tumor; Kynesha (Toy Connor) continues hiding out from her would-be assassin with the help of Benton (Eriq La Salle) and Finch (Michael Michele); and Weaver (Laura Innes) is startled by a spontaneous act of affection -- from another woman. ~ Hal Erickson, All Movie Guide

The ER is the site of several personal crises: the parents of a premature infant implore the staff to save the child, a gay woman (Rosemary Forsyth) is prevented by legal red tape from getting treatment for her comatose life partner, and a homeless patient who refuses to show up for dialysis sparks a quarrel between Benton (Eriq La Salle) and Romano (Paul McCrane). In another development, Carter (Noah Wyle) attends his first AA meeting, where another of his ER colleagues agrees to be his sponsor. Alan Young guest stars as Archie Mellonston, a nursing-home patient who raises a ruckus in the ER when he flashes back to a traumatic incident during the Korean War. Other developments: blacklisted throughout Chicago by the vengeful Romano (Paul McCrane), the unemployed Benton (Eriq La Salle) is saddled with additional problems courtesy of his pugnacious nephew Jesse (Andrew McFarlane); Greene (Anthony Edwards) treats a beauty contestant who was injured in a freak "cosmetic" accident; and Chen (Ming-Na) takes care of a terminal patient whose mother is in a state of denial. Elsewhere, a helpful neighbor brings in an elderly couple suffering from suspicious injuries.
