Cristina (Sandra Oh) and Burke (Isaiah Washington) have trouble summoning enthusiasm over their impending marriage as long as Colin (Roger Rees) remains on the scene. Alex (Justin Chambers) is convinced that he has found the family of the beleagured "Jane Doe" (Elizabeth Reaser). In the first half of a two-part story, no sooner has George's father Harold O'Malley (Geoge Dzundza) nemerged from heart surgery than he must undergo another operation for cancer. Both Meredith (Ellen Pompeo) and Derek (Patrick Dempsey) suffer mightily when Meredith's estranged father Thatcher Grey (Jeff Perry) arrives in town to visit his other daughter Molly and his new granddaughter. While still putting off cashing her huge inheritance check from Denny, Izzie (Katherine Heigl) bonds with Heather, a teenager with a bad attitude, a worse case of spinal curvature, and no money for additional surgery. Meanwhile, Alex (Justin Chambers) and Izzie (Katherine Heigl) wonder what they can do for an encore after their linen-closet rendezvous; a patient (Arlene Tur) suffering from persistent spontaneous orgasms has the staff all a-twitter; and a blast from the past drives yet another wedge between Derek (Patrick Dempsey) and Addison (Kate Walsh) with the unexpected arrival of Dr. Mark Sloan (Eric Dane) in his first series appearance). As Season Three of Grey's Anatomy gets under way, Meredith (Ellen Pompeo) has only a few fleeting moments to ponder her romantic rendezvous with Derek (Patrick Dempsey) at the end of Season Two before Derek and George (T.R. Knight) are quarantined in the same room by a flu epidemic. In another unresolved issue from the previous season, Izzie (Katherine Heigl) is so devastated by the death of heart patient Denny Duquette that she decides to give up her medical career completely. Elsewhere, the wife (Loretta Devine) of surgery chief Webber (James Pickens Jr.) forces him to choose between the hospital and her; and two of the surgeons look after an infant found in a most unexpected place. Still unsettled in matters of the heart, Meredith (Ellen Pompeo) is ill-prepared for a medical emergency. And Izzie (Katherine Heigl) receives an astonishing "reward" from the father (Fred Ward) of the late Denny Duquette. It's the annual Dead Baby Bike Day, in which the bicycle messengers of Seattle participate in a city-wide race. Unfortunately, there's been a spectacular accident, and the ER is filled to overflowing with injured cyclists and pedestrians. Among the casualties is a biker named Viper (Callum Blue), who has spokes protruding from his abdomen--not that this prevents him from hitting on Meredith (Ellen Pompeo), who has enough trouble on her hands fending off Derek (Patrick Dempsey). On a more somber note, anothere of the victims is an unidentified, brain-dead woman, whose organs may end up being harvested for a gay cancer patient (Keith David. It is up to Izzie (Katherine Heigl) to decide whether or not to remove the comatose woman's liver...and, more problematically, to determine how long to wait before doing so. ~ Hal Erickson, All Movie Guide

The interns are dumbstruck by patient Annie Connors (Alex S. Alexander), who checks in with a 65-pound tumor--an all-time record for Seattle Grace. Another patient, Mr. Levangie (Bruce Weitz), suffers from Parkinson's disease, the proper treatment of which sparks a war of words between surgical intern Izzie (Katherine Heigl) and her gimlet-eyed supervisor Bailey (Chandra Wilson). Elsewhere, Bailey warns Meredith (Ellen Pompeo) that her relationship (such as it is) with Derek (Patrick Dempsey) may cost both of them their jobs; and Izzie offers advice to George (T.R. Meanwhile, Alex (Justin Chambers) tries to persuade an 18-year-old girl named Nicole (Michelle Ongkingco) to undergo corrective surgery--and at the same time, Nicole gives Alex advice on his relationship with Izzie (Katherine Heigl); George (T.R Knight) finds himself identifying with the long-suffering husband (Reni Santoni) of an overbearing heart patient (Elaine Kagan); and Cristina (Sandra Oh) and Burke (Isaiah Washington) aren't sure whether to let everyone know about what's going on between them. ~ Hal Erickson, All Movie Guide

Everyone seems to be having a Merry Christmas except the people at Seattle Grace. Izzie (Katherine Heigl) can't get into the holiday spirit because she's still p.o.'ed at Alex (Justin Chambers), who for his part is convinced that he'll fail again when he takes his board exams for the second time. Back at the hospital, Bailey (Chandra Wilson) maintains her backbreaking schedule despite her pregnancy; George (T.R. Knight) can't help but feel that his patient's obnoxious relatives are the root cause for her gastric ulcer; and Cristina (Sandra Oh) and Burke (Isaiah Washington) have a theological argument while trying to persuade a 10-year-old to undergo a heart transplant. Finally, though Derek (Patrick Dempsey) and Addison (Kate Walsh) have reconciled for the umpteenth time, it doesn't last long--a fact that doesn't make Addison happy, but which may turn out to be the best Christmas present that Meredith (Ellen Pompeo) ever had. ~ Hal Erickson, All Movie Guide

A power outage at Seattle Grace traps George (T.R. Knight) in an elevator with a seriously wounded police officer (Daniel Bess), forcing him to perform emergency surgery. The outage also causes another patient to lose his "pain medication"--to be exact, his collection of video porn--forcing Cristina (Sandra Oh) to do some clever improvising. Elsewhere, after trying to convince a religious zealot to authorize surgery for his daughter, Meredith (Ellen Pompeo) has a showdown with Derek (Patrick Dempsey). Giles (Anthony Stewart Head) arrives at the Magic Box and mystically shackles the grief-mad Willow (Alyson Hannigan), then hears from Buffy (Sarah Michelle Gellar) about everything that's gone down since his departure (see "Tabula Rasa"). Their joyful reunion is interrupted, though, when Willow mystically compels Anya (Emma Caulfield) to free her. Luckily, Giles has come to end the Wiccan's rampage, and he's armed with the magicks of an entire coven. They battle epically, but soon Willow drains Giles of his powers and leaves him near-dead. Elsewhere, Xander (Nicholas Brendon) and Dawn (Michelle Trachtenberg) are on the run with surviving super-nerds Jonathan (Danny Strong) and Andrew (Tom Lenk). Willow dispatches the magical equivalent of a guided missile to kill the villains and their reluctant protectors. Buffy manages to save her friends, but in the confusion, Jonathan and Andrew escape; Buffy and Dawn end up in a cavern full of zombies. As for Willow, the influx of Giles' power puts her magically in touch with the pain and suffering of the entire planet; she resolves to burn the Earth to a cinder. Anya warns Xander of the impending apocalypse and he rushes to the rocky bluff where Willow is raising the malign temple of an ancient doomsday cult. Despite Willow's vicious attacks, Xander showers her with love and acceptance, which eventually drains her of black magic and restores her humanity. Meanwhile, after battling side by side with her sister, Buffy finally realizes that she's glad to be alive again. And in a dark cave somewhere in Africa, Spike (James Marsters) completes his quest to be returned to his former self. But his demon taskmaster doesn't remove the chip in Spike's head -- he returns the vampire's soul.
